FDA Rewards and Recognition Program Nomination Form 3450

Clock on a pile of moneyThe FDA recently notified NTEU that it has plans to revise the Rewards and Recognition Program Nomination Form 3450 for incentive awards. Per the notice, the agency proposes the following changes to the form:

  • Replace Social Security Number with CaphHR Employee ID, and remove all references to Social Security Number;
  • Remove language “only management may nominate/approve time-off awards”;
  • Expand the justification portion to accommodate up to one page. This text field will expand when you write more and all the text will appear when printed; and
  • Create an “Add Page” button to allow for nominators to include all group members and associated information for a group nomination. No supplemental document or attachment will be required. All relevant information will be available on the same form and will print with the nomination.

Please note that Article 27, Section 5 B 2 c, of the CBA, gives incentive awards committees the authority to develop nomination forms.
