Incentive award nomination form issue resolved

Please note that Article 27, Section 5, of the 2014 HHS-NTEU Consolidated Collective Bargaining Agreement (CBA) authorizes joint labor-management incentive awards committees to develop a process for submitting nominations for awards and recognition including guidelines, criteria, forms, and  information. Therefore, these committees may choose to create a form other than form FDA 3450 to submit nominations. Please contact the incentive awards committee in your laboratory or district office to find out the specific process and nomination form they agreed to use.

If your laboratory or district office incentive awards committee chooses to use form FDA 3450 for nominations, please note that this form erroneously states that, “only management may nominate/approve time-off awards.” In fact, any employee can nominate any employee for an incentive time-off or cash award. This also means that any employee can self-nominate! I recommend that you make sure your accomplishments are recognized by self-nominating.
